Congrats on making it this far!

You’ve completed your comprehensive fatigue assessment and built your care plan with our team. Now we move forward into treatment.

This email contains your Integrated Fatigue Care (IFC) Enrollment Agreement. Once signed, you’ll be able to book your weekly group supervised exercise session through our patient portal.


A second email is coming

You’ll also receive a separate Equal Care Contribution (ECC) Program Membership Agreement. ECC is optional. If you choose to enroll, it unlocks:

  • Additional weekly group supervised exercise sessions
  • Follow-up CPETs to track your progress
  • Updated rehabilitation prescriptions as you advance

You can enroll in IFC only, or in both IFC and ECC. The choice is fully yours.


What happens next

  1. Sign the IFC Enrollment Agreement below.
  2. Review the ECC agreement when it arrives. Sign it to enroll; set it aside if you’d prefer IFC alone.
  3. Book your first appointments through your patient portal.

The Cancer Fatigue Services Team

Dr. Scott Adams is a physiologist and scientist with over 18 years of experience in precision exercise medicine – designing, delivering, and testing exercise-based interventions grounded in advanced physiological assessment. Prior to founding Peak Precision Health, Scott was a scientist and oversaw a cardiopulmonary exercise testing lab at the University Health Network. He was the inaugural co-chair of the Cardio-Oncology Rehabilitation & Exercise working group of the International Cardio-Oncology Society. He completed his PhD at the University of Alberta (Edmonton, Alberta) and two postdoctoral fellowships at Memorial Sloan Kettering Cancer Center (New York, New York) and the University of Toronto (Toronto, Ontario).

As a clinician, he co-founded and co-led multidisciplinary health and exercise programs across major academic medical centres. As a scientist, Scott has worked with top research teams within hospital, university, and community settings across North America. His research focused on using cardiopulmonary exercise testing and metabolic assessment to predict, identify, minimize, and treat some of the most common and debilitating consequences of chronic disease – including cardiovascular dysfunction, skeletal muscle impairment, metabolic inefficiency and fatigue.

Matthew Dhanushan completed his Bachelor of Arts at York University in Kinesiology & Health Sciences and a Master’s degree in Fitness Science at York University, with a concurrent certificate in Exercise Assessment and Prescription. Since graduating, he has been working as an exercise physiologist developing expertise in cardiopulmonary exercise testing (CPET). Working in various research and clinical settings, he has gained a strong understanding of the complex relationship between exercise and the human body.

Matthew has been an integral part of research teams at Sunnybrook Hospital and York University. He has acquired important experience working with various clinical populations, including individuals living with chronic health conditions, and has gained a strong understanding of the nuances and challenges faced by people living with chronic illnesses.

From 2022-2024, he worked as a teaching assistant and guest lecturer at York University, where he made an impact in supporting the educational development of students in the Health Sciences and Life Sciences programs. He supported courses in Human/Exercise Physiology, Anatomy, and Nutritional Sciences.

Matthew is passionate about exercise and advocates its benefits in managing chronic disease symptoms and improving quality of life. He is compassionate and is driven to deliver personalized, evidence-based interventions to positively impact outcomes.
